Job Description

TriSource is seeking a compassionate and organized Registered Nurse (RN) to join our team in Residential & Community-Based Services. In this role, you will function as an autonomous Program Nurse, providing essential medical oversight and guidance for individuals with intellectual and developmental disabilities. You will work closely with an interdisciplinary team to ensure the highest standards of health, safety, and person-centered care.

Benefits Qualifications License:
Current Pennsylvania Nursing License (RN) in good standing.
Education:
Diploma, Associate's, or Bachelor's Degree in Nursing.
Experience:
2 years of experience in acute care, long-term care, or IDD services preferred. New graduates are welcome to apply; we provide training!
Requirements:
• Valid driver's license with a clear 3-year driving record. Ability to lift/move up to 50 lbs (assisting with client transfers). Satisfactory completion of a background check, child abuse clearance, and drug screen.
Key Responsibilities Clinical Oversight:
Coordinate and oversee healthcare services, perform health assessments, and identify health/safety concerns.
Goal Management:
Develop and implement individualized healthcare goals and person-specific protocols.
Team Collaboration:
Act as the medical lead for the interdisciplinary team, communicating vital health information to families and providers.
Medication & Equipment:
Supervise medication management (distribution, storage, disposal) and coordinate medical equipment needs.
Documentation:
Maintain accurate electronic health records, transcribe physician orders, and complete required nursing notes.
Direct Support:
Attend specialty medical appointments, respond to emergencies, and provide health-related training to staff and individuals. Apply Today!
