Job Description

Position Summary The Registered Nurse (RN) provides overnight nursing care within a state-operated long-term care and skilled nursing facility serving adults with chronic medical, psychiatric, behavioral health, and dementia-related conditions. The RN is responsible for monitoring resident status, administering medications and treatments, responding to medical and behavioral emergencies, supervising support staff, and ensuring a safe and therapeutic environment during overnight hours. Essential Duties and Responsibilities Provide direct nursing care to residents with chronic medical and behavioral health conditions during overnight hours. Administer prescribed medications, treatments, and therapeutic interventions accurately and safely. Monitor residents for changes in physical, mental, or behavioral status and communicate concerns to providers and oncoming staff. Perform ongoing assessments, safety rounds, and overnight monitoring of residents with dementia, psychiatric diagnoses, and complex medical needs. Respond promptly to medical and behavioral emergencies. Supervise nursing assistants and direct care staff assigned to the night shift. Complete documentation, nursing notes, medication records, and incident reports accurately and timely. Collaborate with interdisciplinary healthcare teams to support individualized resident care plans. Maintain compliance with infection control, safety standards, and facility policies. Promote a calm, secure, and therapeutic overnight environment for residents. Qualifications Current Registered Nurse (RN) license in the Commonwealth of Pennsylvania. Graduation from an accredited nursing program. Strong clinical assessment, critical thinking, and communication skills. Experience in long-term care, behavioral health, skilled nursing, geriatrics, or dementia care preferred. Ability to work independently and effectively during overnight operations. #
PVDR Pay:
$43.00 - $46.00 per hour
