Job Description

Registered Nurse (RN) - Home Health & Hospice At Personal Home Healthcare and Hospice we provide care needed in the patient's home setting in order to improve health, independent functioning and prevent hospitalization. We are seeking a full-time Registered Nurse (RN) to join our healthcare team to service patients in Monroe county and surrounding areas. General schedule is Monday through Friday daytime hours. Responsibilities:
Assess and establish patient treatment plans in conjunction with physician orders and patient preferences Help develop, coordinate and implement the Nursing Care Plan in conjunction with the multi-disciplinary careteam Provide skilled nursing care as ordered by the physician to patients in their home setting Monitor and report changes in patient symptoms or behavior Communicate with collaborating physicians and other healthcare providers regarding patient care Educate patients about health maintenance and disease prevention Maintain timely and accurate patient medical records Provide advice and emotional support to patients and their family members Participate in on-call duties as needed per agency policy
Requirements:
Must be currently licensed in Pennsylvania as an RN Minimum one year home health and OASIS experience preferred but willing to train the right candidate Reliable with good communication, organization, and clinical skills Have a caring, compassionate, personable and collaborative attitude Familiarity with medical software and equipment A valid driver's license in the state of Pennsylvania Because our team is the foundation of the care and service we provide, we look for highly skilled, caring and personable team members.
