The role involves providing management and delivery of patient care plans, supporting patients, and consistently meeting critical deadlines for charting. The ideal candidate will have an RN license in the state of work, two years of RN experience with hospice experience preferred, and the ability to self-manage time and schedule. Key Responsibilities

Schedule:

Monday-Friday, 8am-5pm and one weekend on-call every 5-7 weeks Friday, Saturday, & Sunday, 8pm-8am; 2 holidays/year Travel

• Occasional local travel Qualifications Required Associate degree or higher Hospice (2+ years) 1+ years of experience in ANY of the following: Electronic Medical Record (EMR) Other EMR TruChart EHR Valid Registered Nurse (RN) in Pennsylvania (PA) Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all 3)?
