Registered Nurse - RN - Home Health Care

OSPTA Home Health Care seeking a full time Licensed Registered Nurse to provide services to homebound patients in West Mifflin and surrounding areas.

SUMMARY OF DUTIES

Able to perform complete comprehensive assessment on patients in their home. To develop a plan of care to meet the patient's needs based on the comprehensive assessment. To report findings to referring physician in order to coordinate care. To provide care as needed and/or supervise care provided by the HHA. To delegate duties to the HHA when appropriate. Able to communicate with staff and physicians regarding patient progress. Able to complete required documentation within company policy. Able to work independently and make decisions related to patient care. Perform assessments within 24-48 hours of receiving referral. •Full job description available upon interview RN Qualifications and Skills Graduate of an approved registered nursing program; licensed to practice, in the state in which they practice; have good oral and written communication skills; be of excellent health and possess a high energy level; demonstrate a commitment to excellent patient care and customer service. 1 yr of RN experience required Home care experience preferred but not required Preferred Experience with the following: Wound care including wound vacs, Phlebotomy, PICC lines and ports Strong computer and technology skills Excellent organizational and communication skills Ability to work independently and within a multidisciplinary team With more than 20 years of service in Western
