Case Management Nurse (must be located in Iowa City, IA or Waterloo, IA area) Job Description The Case Management Nurse delivers comprehensive, client-centered case management services across the continuum of care, ensuring that each individual receives the most appropriate level and type of health, medical, financial, legal, and psychosocial support. This role blends direct client interaction with care coordination, education, advocacy, and resource navigation to promote wellness, manage acute and chronic conditions, and improve overall quality of life. At least half of the nurse's time will focus on direct case management services guided by established case management standards and accreditation guidelines. What You'll Do Manage a caseload of clients, helping coordinate care and supporting them through their healthcare journey Build personalized care plans and stay connected with clients to ensure they're getting the support they need Advocate for clients and help remove barriers to care Connect clients with community resources and services Work closely with healthcare providers and community partners Keep documentation accurate and up to date Why This Role Stands Out 100% remote position (must be located in the Iowa City or Waterloo area) Company vehicle provided Monthly stipends: $120 for phone + $67 for internet Quarterly bonus potential Competitive salary: $70K-$105K , depending on experience Opportunity to make a real impact in patients' lives every day What We're Looking For Active RN license (required) Case management or care coordination experience is a plus, not required Strong communication and relationship-building skills A passion for helping others and improving patient outcomes Additional Skills & Qualifications Experience with vocational case management, including job analysis, vocational assessments, job development, job placement, and life care planning, is beneficial. Experience coordinating ergonomic evaluations, work hardening, and transitional work programs is an advantage. Familiarity with professional support organizations such as the American Heart Association, American Lung Association, U.S. Department of Health and Human Services, American Cancer Society, and state health departments is helpful. Comfort working with clients facing complex psychosocial issues, including chronic illness, disability, and various forms of abuse, is desirable. Commitment to ongoing professional development and participation in quality improvement projects. Work Environment This role operates across a continuum of care and may involve coordination with hospitals, clinics, community agencies, and home-based services. The Case Management Nurse spends a significant portion of time in direct client contact, which may include in-person interactions, phone calls, and virtual communication. The position emphasizes collaboration with interdisciplinary teams, community organizations, and various healthcare and insurance entities. The work environment supports ongoing training in ethics, regulatory compliance, and best practices in case management. The schedule and setting may vary depending on client needs and organizational requirements. The organization offers supportive resources for communication, including a monthly cell phone allowance and internet allowance, as well as eligibility for quarterly bonuses tied to performance and outcomes.
