Our goal at American Pain Consortium is to support our clinics that help patients seeking relief from pain, numbness, and tingling sensations that are often related to neuropathic pain or neuropathy, among many other forms of chronic pain. Even patients who have tried other pain treatments without success often benefit significantly from safe, effective treatment at American Pain Consortium. Our clinical multidisciplinary approach to care delivers the correct intervention to every patient, every time through various pain management procedures including injections, open surgical cases with general anesthesia, physical therapy, and psychological services. Our Providers focus on helping individuals improve functionality to restore a more meaningful lifestyle. APC is seeking an individual who is eager to learn, develop, and grow and help us contribute to our overall mission to empower Providers to deliver exceptional patient care. What will you do? This position is responsible for successful implementation, management, support, and operation of a collaborative care and case management model that serves as an extension of the clinical staff for patients with implanted intrathecal pumps. Essential Duties & Responsibilities Documents patient care in accordance with facility policy/procedures and state standards. Manage care for patients with implantable pain pumps during intervals between office visits and medical refills. Answer patient questions and follow-up on patient concerns, track their care, and symptom management. Supports overall well-being of patient by maintaining consistent contact with providers overseeing other aspects of patient care to ensure best patient outcome. Monitor and timely prompts for order of additional medication needed for upcoming implantable pump refills. Track inventory with all appropriate documentation required for controlled substances. Track and document patient concerns regarding medication being dispensed from the implantable pump. Follow all guidelines or escalation of care concerns with prescribing physician. Facilitate scheduling of patients with implantable pumps for services throughout the organization's entities while balancing departmental resources, utilization requirements, and addressing physician/patient requests or needs. Responsible to ensure all actions taken and care provided is in compliance with state and federal code and that all necessary patient care is rendered as appropriate. Assure timely and thorough completion of documentation, assists with policy development, or updating as requested. Review patient charts and assess documented plan of care for application of consistent best care practices. Assess current therapies and treatments provided to patients. Recommend alternative or additional services that may support better patient outcomes or overall addresses patient concerns. Other duties as assigned What do we offer?
