  • Field-Based Case Manager (RN / Behavioral Health Clinician)
  • LTSS Join Aetna's Long-Term Services and Supports (LTSS) team in a dynamic, field-based Case Manager role where you can make a meaningful difference in the lives of members throughout Virginia.
This position is ideal for a compassionate Registered Nurse or Behavioral Health Clinician who is passionate about delivering personalized, community-centered care and thrives in a role that blends autonomy, travel, and collaboration. As a Case Manager, you will take the lead in assessing, planning, and coordinating comprehensive care for members with diverse medical and behavioral health needs. Through in-person visits (up to 75% travel), you'll build strong relationships, gain deeper insights into members' environments, and create tailored care plans that promote both immediate health improvements and long-term wellness . You will play a critical role in integrating services, addressing barriers to care, and connecting members with appropriate resources across healthcare and community systems. By working closely with interdisciplinary teams, providers, and support networks, you'll ensure care is proactive, coordinated, and aligned with each member's unique goals. What makes this role stand out: Field-based work that allows direct, meaningful member interaction Opportunity to impact both medical and behavioral health outcomes Autonomy to manage cases while collaborating with a supportive care team Work-from-home flexibility with structured weekday hours (Monday-Friday, 8:00 AM-5:00 PM) Key responsibilities include: Conducting in-depth, face-to-face assessments to identify member needs Developing and implementing individualized care plans Coordinating medical, behavioral, and community-based services Monitoring progress and adjusting care strategies to improve outcomes Advocating for members to ensure access to high-quality, appropriate care Required Qualifications Must reside in Norfolk, VA area (Chesapeake, Portsmouth, Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy RN/BH with current unrestricted Virginia state licensure required. 3 years Clinical Experience (Preferably in Medical-Surgical Nursing, Behavioral Health, Long term Care, etc.) 1+ years Managed Care experience. Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding. Preferred Qualifications Case Management in an integrated model preferred 5 years clinical practice experience Effective communication skills, both verbal and written. Bilingual in English/Spanish Education Registered Nurse
  • Associate's Degree Required. Bachelor's Degree of Science Preferred Licensed BH Clinician
  • Master's Degree Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00
  • $129,615.
00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
