Job Description
Make a Meaningful Difference with Blue Water Home Care & Hospice At Blue Water Home Care & Hospice , we are dedicated to providing compassionate, patient-centered hospice care that brings comfort, dignity, and peace to patients and families. We are seeking a caring and experienced Community Hospice Registered Nurse (RN) to join our interdisciplinary team. What You'll Do Conduct hospice admissions and comprehensive patient assessments. Develop, implement, and update individualized plans of care. Provide skilled nursing care and symptom management according to physician orders. Educate patients, families, and caregivers on disease progression, hospice services, and end-of-life care. Coordinate care with physicians, hospitals, skilled nursing facilities, and interdisciplinary team members. Assess caregiver needs and provide ongoing support and guidance. Supervise LPNs, hospice aides, and paraprofessional staff in accordance with regulatory requirements. Complete accurate and timely clinical documentation using RXNT and Enclara. Participate in interdisciplinary team meetings, quality improvement initiatives, and on-call coverage. Qualifications ✔ Current Michigan RN License (unencumbered) ✔ Graduate of an accredited nursing program ✔ Current BLS/CPR Certification ✔ Valid Driver's License, Auto Insurance, and Reliable Transportation ✔ Hospice, Home Health, Palliative Care, or Related Nursing Experience Preferred ✔ Strong assessment, communication, and care coordination skills Why Blue Water Home Care & Hospice?
