We build from the resources of the community to plan and direct services to meet the needs of our patients and their families. Responsibilities Collaborate with physicians and other professionals to plan care Educate patients and their families on prevention and healthy habits Perform routine procedures, adjust and administer medications and provide treatments according to physician's orders Work with caregivers to plan and execute excellent patient care Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate. Participates in on-call duties as defined by the on-call policy Supervise and train LPN's and Home Health aides as appropriate

Skills:

Licensed Registered Nurse (RN) in Idaho and CPR certification Home Health Nursing experience preferred Hospice Nursing experience preferred Must have current drivers license, auto registration and insurance Must be able to pass a background check Management experience not required, but will be able to work without supervision and will be responsible for supervising home health aides. Must have excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Must be willing to share On Call on a rotation basis #
