Position Details Contract per visit — flexible schedule South San Diego County service area Consistent case flow with opportunity to grow your caseload Responsibilities Conduct skilled physical therapy evaluations and treatment visits in the patient's home Complete and submit OASIS assessments accurately and on time Develop and implement individualized plan of care in coordination with the physician and care team Provide gait training, strengthening, balance, transfer training, fall prevention, and ADL retraining Educate patients and caregivers on home exercise programs and safe functional mobility Maintain timely documentation in compliance with Medicare and agency standards Communicate clinical updates and changes in patient condition to the clinical team Requirements Active California Physical Therapy license in good standing Minimum 1 year of experience OASIS competency required — must be able to complete independently CPR certification current Reliable transportation and valid California driver's license Strong documentation and communication skills Why Dependable Nursing 20+ years serving San Diego County Supportive clinical team and responsive office staff Consistent referral pipeline — cases available now Flexible scheduling — you set your availability Prompt pay per visit
