Psychiatric Nurse Practitioner (Hybrid) -
Skagit County, WA Shifts:
Days (Mon-Fri) The schedule includes day shifts with 3 clinical days, 1 administrative day, and 3 days off, along with rotational on-call weekends. On-call duties are conducted via phone only
Job Description:
We are currently seeking a Psychiatric Nurse Practitioner specializing in the care of homebound geriatric patients in the SKAGIT and WHATCOM county regions. The Ideal candidates should have a passion for treating the geriatric population and should be well-versed in the common conditions that are presented in this demographic, such as dementia, cardiovascular disease, endocrinological conditions, neuro degenerative diseases, to name a few.
Roles and Responsibilities:
  • Conduct detailed patient assessments, including medical history, physical examinations, and mental health evaluations.
  • Collaborate with the treatment team to develop and implement individualized care plans, emphasizing evidence-based interventions and medication-assisted treatment (MAT).
  • Prescribe and manage medications for addiction and other health conditions, monitoring efficacy and side effects.
  • Provide ongoing patient education and counseling on substance abuse, health maintenance, and disease prevention.
  • Medical management is conducted in collaboration with other health care team members, such as clinical staff and other patient care team members to ensure a coordinated and comprehensive approach to patient care.
Qualification/Experience:
  • Master's degree or Doctorate Degree as a Psychiatric Nurse Practitioner
  • Minimum 2-3 Years of experience working with Geriatric Population
  • New Graduates are also accepted
Licenses/Certifications:
  • Current license to practice as a Psychiatric Nurse Practitioner in the State of Washington
