Location Requirement:
must reside in one of the following areas: North Miami
  • Aventura
  • Hialeah
  • Miramar
  • What You'll Do:
  • Manage a caseload of members through telephonic and in-person visits
  • Conduct assessments and develop individualized care plans
  • Coordinate services including prior authorizations, provider collaboration, and community resources
  • Support members with chronic conditions, medication reviews, and care planning
  • Advocate for patients while ensuring compliance and optimal outcomes
  • Identify opportunities to improve care quality and reduce costs
  • What We're Looking For:
  • Fluent Bilingual - English & Spanish (required)
  • Bachelor's degree required (healthcare-related field preferred, but not required)
  • Case management experience required
  • Proficiency in Microsoft Office (Excel, Word, etc.)
  • Ability to travel 50-75% locally for in-person visits
  • Preferred Experience:
  • Long-term care or managed care background
  • Experience working with complex patient populations
  • Additional Details:
  • Training conducted remotely via Microsoft Teams
  • Travel includes home visits, assisted living facilities, and nursing homes

  • Pay Details:
  • $35.
