Required Qualifications Candidates must possess ALL of the following qualifications to be considered: Active Illinois Registered Nurse (RN) license in good standing Minimum three (3) years of recent Medicare-certified home health experience Extensive OASIS-E knowledge and experience Advanced proficiency with Axxess Home Health software Strong understanding of Medicare Conditions of Participation and home health regulatory requirements Position Summary Luxe Home Health is seeking an experienced Home Health Quality Assurance RN to support our Illinois operations. This is a contract, primarily remote position responsible for reviewing clinical documentation, auditing OASIS assessments, ensuring regulatory compliance, and supporting quality improvement initiatives. The ideal candidate is a highly skilled home health clinician who can work independently, identify documentation deficiencies, recognize compliance risks, and provide meaningful recommendations to improve clinical quality and patient outcomes. While this position is primarily remote, the selected candidate must be available to attend in-person meetings at our Des Plaines office one to two times per week to review workload, discuss findings, and collaborate with leadership on quality initiatives. Essential Duties and Responsibilities Quality Assurance & Compliance Conduct comprehensive clinical chart audits to ensure accuracy, completeness, and regulatory compliance Review OASIS assessments and supporting documentation for quality and consistency Monitor compliance with Medicare Conditions of Participation, Illinois regulations, and agency policies Identify documentation deficiencies and compliance concerns and provide recommendations for correction Assist with quality improvement and performance improvement initiatives Support agency readiness for audits, surveys, and accreditation reviews Track, trend, and report quality findings and documentation outcomes Clinical Documentation Review Review Plans of Care, physician orders, visit notes, recertifications, transfers, resumptions of care, and discharge documentation Ensure documentation supports medical necessity, homebound status, and skilled need requirements Verify timely completion and submission of clinical documentation Monitor documentation trends and identify opportunities for clinician education and process improvement Promote consistency and accuracy throughout the clinical record Collaboration & Operational Support Collaborate with agency leadership to improve documentation quality and regulatory compliance Participate in regular meetings to review workload, findings, and quality metrics Assist in developing and maintaining quality assurance processes and best practices Support ongoing efforts to improve clinical outcomes and operational efficiency Preferred Qualifications Previous Quality Assurance, Clinical Review, Clinical Manager, Clinical Team Manager, or Director of Nursing experience in home health Experience preparing for CHAP, ACHC, or state regulatory surveys Experience with quality improvement and performance improvement programs Knowledge, Skills, and Abilities Exceptional attention to detail Strong critical thinking and clinical judgment Excellent written and verbal communication skills Strong organizational and time-management abilities Ability to work independently with minimal supervision Ability to manage multiple priorities and meet deadlines Working Conditions Primarily remote work environment Required attendance at the Des Plaines office one to two times per week Flexible schedule based on workload and agency needs Contract engagement anticipated to continue based on ongoing agency needs; no predetermined end date has been established Why Join Luxe Home Health?
