Home Health Registered Nurse (RN) - Sherman, TX Make a difference where it matters most — in the patient's home. Chrysalis Home Healthcare is seeking a compassionate, skilled Registered Nurse (RN) to join our growing Home Health team in Sherman. If you value autonomy, meaningful patient relationships, and flexible scheduling, this role offers the opportunity to provide high-quality, patient-centered care in a supportive and growing agency. What You'll Do Perform comprehensive patient assessments in the home and develop individualized plans of care Administer medications and treatments as ordered, including injections and venipuncture Provide patient and caregiver education on disease management, medications, infection control, and safety Deliver skilled nursing care including wound care, catheter care, and chronic disease management Monitor patient progress, recognize changes in condition, and communicate effectively with physicians and the care team Collaborate with interdisciplinary team members to ensure coordinated, high-quality care Maintain accurate and timely documentation in compliance with Medicare and regulatory standards What We're Looking For Active Registered Nurse (RN) license in the state of Texas Home health or primary care experience preferred Strong assessment, critical thinking, and clinical decision-making skills Proficiency with injections, venipuncture, wound care, and catheter care Knowledge of infection control standards and best practices Ability to work independently and manage multiple patients efficiently Excellent communication and interpersonal skills Experience with Kinnser/WellSky preferred Willingness to travel within a 20-25 mile radius of our Sherman office Compensation & Schedule Pay per visit Flexible scheduling Supportive leadership and clinical team Growing census with opportunities for consistent visits Why Chrysalis Home Healthcare We are committed to delivering high-quality, compassionate care while supporting our clinicians with clear communication, strong leadership, and room to grow.
