Job Description

The Home Hospice After Hours On-Call Nurse is the primary patient contact during after-hours and weekends with support from the Interdisciplinary Team as whole. This nurse is experienced in nursing, with emphasis on triage and emergency management. The On-Call Hospice Nurse strives to meet the needs of individual and families within their homes by triaging needs as well as completing visits as needed/directed by clinical leadership.
Essential Job Functions/Responsibilities Patient Care:
Triages via the phone and completes visits for hospice patients as needed during on-call shifts. (Shifts are currently Friday 5pm to Monday 8:30am as well as 5pm to 8:30am the following Monday, Tuesday, Wednesday, and Thursday nights). Report is communicated to the on-call RN after 4:30pm on weekdays. Morning Report is completed at 8:40am via phone call or messaged to clinical leadership prior to 8:40am. Back Up Nurse support is scheduled for weekend on-call shifts and as needed during additional time periods. Communicates with physician on-call at night for orders as well as medical support as appropriate. Initiates appropriate nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and comfort care. Orders medication and equipment while on-call if the need is unable to be addressed the next day by case manager or clinical leadership. Completes death and post-mortem visits that occur during on-call shift. Connects with funeral homes and appropriate parties. On occasion, the on-call nurse may be scheduled for an after hours or urgent admission visit. This will be conducted with support from the clinical leadership or on-call administrator team.
Communication:
Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Participates in Morning Reports and provides relevant information to daytime case managers as well as the interdisciplinary team. Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required. Communicates appropriately with on-call administration and clinical leadership to meet the afterhours needs that may arise. Teaches the patient and family/caregiver self-care techniques as appropriate.
Additional Duties:
Assumes responsibility attendance and participation in continuing education and in-service classes. Actively participates in quality assessment performance improvement teams and activities. Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
Qualifications:
Graduate of an accredited school of nursing 1-2 years experience in nursing (preferred) 1-2 years of recent experience in hospice (preferred) Michigan nursing license, CPR certification, valid driver's license Clear driving records with no more than 3 moving violations in the last three years Excellent communication skills
