Job Description
Job Overview Join our compassionate healthcare team as a Hospice Licensed Vocational Nurse (LVN), where your expertise will make a meaningful difference in the lives of patients facing end-of-life journeys. In this vital role, you will provide specialized nursing care, support families, and collaborate with interdisciplinary teams to ensure comfort, dignity, and quality of life for our patients. Responsibilities Deliver comprehensive nursing assessments, including vital signs, symptom management, and patient evaluations in home or facility settings Administer medications, injections, IV infusions, and other treatments following strict protocols and documentation standards Monitor patients' conditions closely, recognizing signs of distress or deterioration and communicating promptly with physicians and family members Educate patients and families on disease progression, pain management strategies, medication administration, and comfort measures Coordinate care plans with interdisciplinary teams such as social workers, chaplains, and physicians to ensure holistic support Maintain accurate medical documentation within electronic health record (EHR) systems like Epic or Cerner while complying with HIPAA regulations Assist with discharge planning, symptom control, and providing emotional support to patients and their loved ones during end-of-life care Skills Proven experience in hospice or critical care nursing environments with familiarity in medical-surgical, ICU, or emergency medicine settings Strong knowledge of medical terminology, physiology, and diagnostic evaluation techniques including ICD-10 coding and medication management Skilled in patient assessment, vital signs monitoring, IV insertion/infusion therapy, catheter care, and sterile techniques Proficiency with EMR/EHR systems such as Epic or Cerner for accurate medical documentation and record keeping Ability to work effectively with diverse populations including individuals with developmental disabilities, dementia (Alzheimer's and memory care), and those requiring complex pain management Excellent communication skills for triage calls, family education, and interdisciplinary collaboration Knowledge of infection control practices and emergency response procedures in acute care settings such as Level I Trauma Centers or hospitals Join us in providing exceptional hospice care that honors each patient's dignity.
