MUST HAVE HOSPICE CARE EXPERIENCE
Job Summary Community Care Hospice OC is a new hospice agency located in Dana Point, CA seeking compassionate and dedicated Hospice Nurses to join our team! The ideal nurse will provide high-quality, skilled nursing care to patients in a hospice setting, ensuring comfort and dignity during their end-of-life journey. This role requires a strong commitment to patient-centered care, effective communication skills, and the ability to work collaboratively with interdisciplinary team. Duties Deliver comprehensive nursing care to hospice patients, focusing on pain management and symptom control. Conduct thorough assessments of patients' physical, emotional, and spiritual needs. Maintain accurate medical documentation in compliance with regulatory standards. Collaborate with nursing team, social worker, and other healthcare professionals to develop and implement individualized care plans. Provide education and support to patients' families regarding end-of-life issues and care options. Administer medications and treatments as prescribed, including airway management and spinal taps when necessary. Assist with family planning discussions as needed to support patient wishes. Utilize electronic health record systems such as Athenahealth for documentation and communication purposes. Experience Previous experience in a HOSPICE nursing role is
REQUIRED.
Familiarity with hospice documentation practices is also essential. Rates can range from $60.00 to $250.00 (per visit) and is dependent on the type of visit being provided.
