POSITION OVERVIEW
Hybrid RN Case Manager role focused on coordinating care for aging adults living independently at home Community-based nursing opportunity centered around care planning, continuity of care, and service coordination No bedside nursing responsibilities required Hybrid structure combining community patient visits with remote follow-up and care coordination work Mission-driven senior care environment with strong interdisciplinary collaboration

SCHEDULE

COMPENSATION & BENEFITS

ROLE RESPONSIBILITIES
Develop individualized patient care plans Coordinate community-based services and ongoing care needs Conduct patient visits within community and home settings Manage follow-up and care coordination remotely Collaborate closely with interdisciplinary care teams Support aging adults in maintaining safe independent living
