POSITION OVERVIEW
Hybrid RN Case Manager role focused on coordinating care for aging adults living independently at home Community-based nursing opportunity centered around care planning, continuity of care, and service coordination No bedside nursing responsibilities required Hybrid structure combining community patient visits with remote follow-up and care coordination work Mission-driven senior care environment with strong interdisciplinary collaboration

SCHEDULE
Monday-Friday schedule 8:00 AM-4:00 PM hours Three community-based days weekly Two remote workdays weekly No nights No weekends No call responsibilities
COMPENSATION & BENEFITS
Base salary: $84,000-$108,000 annually based on experience Health insurance with 75% employer-paid premium Dental and vision insurance 26 combined PTO days 13 paid holidays Mileage reimbursement 403(b) retirement plan 3 CME days with $3,000 reimbursement License renewal reimbursement Life insurance coverage Disability coverage Flexible Spending Account (FSA) plans Legal assistance benefits
ROLE RESPONSIBILITIES
Develop individualized patient care plans Coordinate community-based services and ongoing care needs Conduct patient visits within community and home settings Manage follow-up and care coordination remotely Collaborate closely with interdisciplinary care teams Support aging adults in maintaining safe independent living

Requirements:
Active Massachusetts RN license
REQUIRED
Minimum 2 years RN experience
