to 5 and Occassional Weekend Find Your Passion and Purpose as an LVN/LPN Offer Based on Years of Experience What You Need to Know Under the supervision of an RN, the Licensed Vocational Nurse (LVN/LPN) provides direct patient care. The LVN/LPN observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse. This position provides direct patient care to pediatric, adolescent, adult and geriatric patients. Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
One year of LVN/LPN work experience
Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records
Must possess and maintain valid CPR certification while employed in a clinical role
Must be a licensed driver who can travel to all business locations
Responsibilities:
As a LVN/LPN, you will: Performs technical nursing functions and direct patient care consistent with the established plan of care and under the supervision of an RN.
Promotes and maintain optimal client care under the supervision of an RN.
Maintains a safe patient environment.
Identifies and communicates significant abnormalities in patients data to case manager and other appropriate home health team members.
Administers prescribed medications and treatments as permitted by law. Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician.
Re-evaluates client's nursing needs and reports findings to Case Manager.
Counsels and instructs the client and family in meeting nursing and related needs.
Prepares clinical and progress notes.
Submits all documentation in a timely manner in accordance with agency policy.
Documentation on patients chart correlates with nursing care plan.
Communicates frequently and works closely with other home care team members.
