Home Matters Caregiving is seeking an experienced and motivated RN to join our growing team. This is a unique opportunity to combine direct client support, caregiver education, and clinical leadership while helping shape the future of home care. We are open to both full-time and part-time candidates .

Full-Time Schedule:

3 days per week in the Portland metro area supporting clients, caregivers, and care quality initiatives 2 days per week remote supporting our corporate clinical team and agency operations If you are looking for a flexible, rewarding nursing role where you can truly impact lives, we'd love to meet you. Responsibilities Conduct in-home client assessments and reassessments Develop, implement, and monitor individualized care plans Collaborate with physicians, families, caregivers, and healthcare professionals Provide clinical oversight and support to clients and caregivers Train, coach, and mentor professional caregivers Lead new caregiver orientation and ongoing skills development Ensure compliance with agency policies, state regulations, and best practices Assist with problem-solving, client concerns, and care coordination Maintain accurate and timely clinical documentation Support quality assurance and clinical initiatives Partner with agency leadership to improve client and caregiver experiences RN Qualifications Active Oregon Registered Nurse (RN) License Current CPR Certification Valid Oregon Driver's License, reliable transportation, and current auto insurance Minimum 2 years of nursing experience (home care, senior care, geriatrics, hospice, home health, or community-based care preferred) Strong assessment, communication, and organizational skills Ability to work independently while collaborating with a multidisciplinary team Passion for caregiver education, mentorship, and senior care Preferred Experience Home Care Home Health Senior Care Geriatric Nursing Care Management Clinical Education Caregiver Training Client Assessments Quality Assurance Case Management Benefits Competitive hourly pay Flexible scheduling Hybrid remote/in-person work environment Paid time off 401(k) Mileage reimbursement Cell phone reimbursement Professional growth opportunities Meaningful work that impacts seniors and caregivers every day Join Our Team At Home Matters Caregiving, we believe exceptional care starts with exceptional people.
