LVN-Intake Specialist

Our mission is to meet individuals exactly where they are, offering a path forward through intensive support, resource navigation, and unwavering advocacy. We are looking for an Intake Nurse. Position Summary We are seeking a compassionate and detail-oriented Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to serve as our Intake Specialist . This crucial role is responsible for conducting comprehensive health assessments for all new residents entering our transitional housing program. The primary goal of this position is to identify and document health needs, ensure the safety of all residents, and connect individuals with appropriate medical resources.

Key Responsibilities Conduct Intake Assessments:

Perform thorough health screenings and assessments for all new residents upon entry into the program. This includes collecting a complete medical history, noting current and past diagnoses, medications, and any chronic health conditions.

Identify Health Risks:

Screen for communicable diseases, potential substance abuse issues, mental health concerns, and other health-related risks that may impact the individual or the community.

Develop Care Plans:

Based on the assessment, create a preliminary health-focused care plan for each resident, outlining immediate needs and recommendations for follow-up care.

Provide Health Education:

Educate residents on their health conditions, medication adherence, and general wellness practices.

Documentation:

Maintain accurate and confidential electronic and paper records of all assessments and interactions in compliance with HIPAA regulations.

Collaboration:

Work closely with the case management team, social workers, and program staff to ensure a holistic approach to resident care.

Resource Referrals:

Refer residents to appropriate external healthcare providers, mental health services, and community resources as needed.

Emergency Response:

Be prepared to respond to and manage medical emergencies that may arise during the intake process.

Qualifications LVN or RN License:

Must hold a current, valid, and unrestricted license to practice as an LVN or RN in the state of California.

Experience:

A minimum of 2 years of experience in a clinical, community health OR hospital setting is preferred. Experience in mental health, substance abuse, or transitional housing is a significant plus.

Skills:

Excellent communication and interpersonal skills. Strong critical thinking and problem-solving abilities. Proficiency in electronic health record (EHR) systems. Ability to work independently and manage multiple tasks effectively. Knowledge of HIPAA regulations and a commitment to maintaining confidentiality. Demonstrated empathy and patience when working with vulnerable populations. Cultural competence and a non-judgmental approach.
