MDS Coordinator

MDS Coordinator (RN Preferred) - Full-Time Hybrid The Admiral at the Lake | Chicago, IL The Admiral at the Lake, a premier Continuing Care Retirement Community (CCRC) located in Chicago, is seeking an experienced MDS Coordinator to join our clinical leadership team. This is a full-time hybrid position offering flexibility while maintaining close collaboration with our interdisciplinary care team. The MDS Coordinator is responsible for the coordination, completion, accuracy, and timely submission of MDS assessments and related clinical documentation in accordance with CMS guidelines, federal and state regulations, and organizational policies. This role plays a key part in reimbursement optimization, regulatory compliance, quality reporting, and resident-centered care planning. What You'll Do Coordinate and oversee all MDS assessments, including Admission, Quarterly, Annual, Significant Change, Medicare PPS, and Discharge assessments. Ensure compliance with CMS, OBRA, Medicare, Medicaid, and state regulatory requirements. Review clinical documentation to ensure accurate coding and reimbursement capture. Monitor assessment schedules, ARDs, transmission deadlines, and regulatory submission requirements. Collaborate with Nursing, Therapy, Social Services, Dietary, and other interdisciplinary team members to support comprehensive care planning. Participate in care plan meetings and quality improvement initiatives. Support survey readiness, audits, reimbursement reviews, and compliance activities. Maintain accurate assessment records and electronic submissions. QualificationsRequired Graduate of an accredited School of Nursing. Minimum three (3) years of experience in Long-Term Care, Skilled Nursing, or Post-Acute Care. Minimum two (2) years of recent MDS Coordinator experience. Extensive knowledge of

MDS 3.0, CMS

regulations, OBRA requirements, and Medicare reimbursement systems. Strong understanding of skilled nursing documentation and quality measures. Exceptional organizational, analytical, and communication skills. Preferred Active RN license. RAC-CT Certification (or ability to obtain within 12 months of hire). Experience in a Continuing Care Retirement Community (CCRC) or senior living environment. Experience with PointClickCare and electronic health record systems. Current BLS Certification.
