Job Summary We ae looking for MDS Coordinator with possiblity of 1-2 remote per week in various locations across RI. In this vital role, you will oversee the comprehensive assessment and documentation of patient care needs within skilled nursing, rehabilitation, or long-term care settings. Your expertise will ensure accurate completion of Minimum Data Set (MDS) assessments, supporting compliance with regulatory standards such as RAI and CMS guidelines. By coordinating closely with interdisciplinary teams, you will facilitate optimal patient outcomes, streamline clinical documentation, and enhance quality of care delivery. This position offers an exciting opportunity to impact patient care through meticulous data management and clinical insight. Responsibilities Conduct thorough MDS assessments in accordance with federal and state regulations, ensuring timely completion and accuracy. Review medical records, clinical documentation, and coding details including ICD-10 codes Collaborate with nursing staff, physicians, case managers, and interdisciplinary teams to gather comprehensive patient information. Utilize EMR (Electronic Medical Record) systems such as PCC to document assessments and update patient records efficiently. Ensure compliance with HIPAA regulations by maintaining confidentiality and security of medical records and documentation. Participate in utilization review and management processes to optimize patient care plans while adhering to Medicare and Mediciad standards. Support clinical documentation improvement initiatives by reviewing records for completeness and accuracy to enhance reimbursement accuracy. Experience Proven experience in MDS coordination within skilled nursing facilities or long-term care environments. Strong background in managed care protocols, Medicare regulations, and utilization review processes. Knowledge of medical coding systems such as ICD-10 and
