Type:

One year contract, possibility for extension

Schedule:

Monday through Friday, 6:45am to 3:30pm. Will be working in office Tuesday-Thursday and remote Monday & Friday On-call rotation after training isplete

Pay:

$40.00 to $44.00 an hour Responsibilities of the

Nurse Case Manager:

• Responsible for coordinating and managing the care of patients withplex medical conditions or disabilities.
• Work closely with patients, healthcare providers, and insurancepanies to ensure that patients receive the appropriate care and support to improve their health oues.
• Assessing patients' medical, psychological, and social needs.
• Developing individualized care plans in collaboration with healthcare providers and patients.
• Coordinating healthcare services, including medical treatments, therapies, and consultations.
• Monitoring patients' progress and adjusting care plans as needed.
• Providing education and support to patients and their families.
• Advocating for patients' needs and rights within the healthcare system.
• Communicating with healthcare providers, insurancepanies, and other stakeholders to ensure coordinated care.
• Evaluating the effectiveness of care plans and making rmendations for improvement.
• Documenting patient assessments, care plans, and interactions.
• Keeping abreast of healthcare regulations, policies, and best practices related to case management.

Qualifications of the

Nurse Case Manager:

• High school diploma or GED
• Active and good standing Registered Nurse in Illinois
• 2+ years of nursing experience
• Occupational Health experience preferred
• Knowledge and understanding of case management work workerspensation, disability, limited duty, safety, industrial hygiene, healthcare plans, regulatory medical requirements and external healthcare resources Why work for Adecco?

Pay Details:
