As a Nurse Case Manager, you will play a crucial role in coordinating, managing, and advocating for the medical care and recovery of injured clients. You will work closely with patients, physicians, legal teams, insurance companies, and rehabilitation providers to ensure optimal outcomes for injured individuals, while also supporting the patient and their family through the recovery process. What You'll Do:
Manage a caseload of auto-injury patients, developing personalized care plans to address the medical, emotional, and financial needs related to the injury. Monitor and assess patient progress throughout the treatment process, ensuring that patients receive timely and effective medical care in line with their treatment plan and legal requirements. Collaborate with multidisciplinary teams, including physicians, physical therapists, occupational therapists, psychologists, and legal representatives, to coordinate care and services. Assist in scheduling and coordinating medical appointments, diagnostic tests, therapy sessions, and follow-up visits. Facilitate communication between healthcare providers, ensuring that patients receive necessary medical tests, treatments, and interventions. Collaborate with insurance companies, legal teams, and third-party vendors to ensure smooth coordination of services and support for claims. Assist in the review of medical records, ensuring proper documentation for insurance claims and reimbursement. Provide detailed reports on patient progress, treatment adherence, and case outcomes to stakeholders, including insurance adjusters and legal representatives. Monitor for any barriers to recovery, such as financial hardship or lack of transportation, and work to address these barriers to ensure timely recovery. Regularly assess the effectiveness of the care plan and make adjustments as necessary.
What You'll Need:
Valid driver's license required Active Registered Nurse (RN) license in Michigan required Bachelor of Science in Nursing (BSN) preferred but not required. Certification in Case Management (CCM) or Medical Case Management preferred but not required. Minimum of 3 years of clinical nursing experience, with at least 1-2 years in case management. Ideally in the auto injury, workers' compensation, or personal injury sector. Experience in coordinating care across multiple healthcare disciplines and managing complex medical cases. Knowledge of auto-injury treatment protocols and the legal/insurance claims process is a plus.
Additional Skills and Competencies:
Excellent communication and interpersonal skills, with the ability to effectively interact with patients, families, medical providers, insurance representatives, and attorneys. Detail-oriented with strong organizational and time-management skills. Ability to work independently as well as part of a collaborative team. Proficient in using electronic medical records (EMR) systems and case management software. Proficient in Microsoft Office Suites (Word, Excel, Outlook, Teams)
Work Environment and Schedule:
Hybrid work environment (combination of office, field visits, telehealth/remote work). Must have or be able to set up a home office workspace free from noise or distractions Occasional travel required to visit patients at medical facilities, rehabilitation centers, or home settings. Conduct home and facility visits (if applicable) to assess the patient's environment Flexibility in hours may be required based on patient needs and case complexity.
