Full Job Description Nurse Case Manager Position Description:
Provides high quality and inclusive healthcare and care coordination to clients with intellectual or developmental disabilities (I/DD) on a caseload. Develops, implements, and evaluates care plans in a community setting, while working with other health care providers and agencies to integrate care. Works a hybrid schedule - 2 days per week in the Lakewood office, and 3 days per week remotely.
Essential Duties:
Coordinates health related services and care plans for PPCH customers. Ensures that services are provided in a manner that respects and acknowledges individuals' self-expression, self-worth, self-reliance, and decision making. Routinely attends and participates in interdisciplinary team meetings for customers on caseload. Develops and maintains and revises comprehensive Care Plans and protocols as needed Recommend updated protocols, staff and provider trainings, or other changes when areas for improvement in the provision of care are identified. Completes home or hospital visits with customers. If currently hospitalized, follow up with hospitalized clients as needed until discharge. Completes health assessments with customers as needed or requested. Processes physician orders and maintain Medication Administration Record (MAR), reviews MARs for accuracy, and updates MARs as needed. Reviews customer appointments due, Care Plans, and MARs on a regular basis, updating as needed and ensuring follow up is completed. Works continuously to improve services and supports using a person-centered approach for the individuals in the IDD community. Contributes to a culture where individuals receiving services are healthy, safe, and valued by others. Models, teaches, and ensures respect for others, including individuals in services, staff, other professionals, guardians and loved ones, and team members. Provides client specific medical training to PPCH contractors and staff. Responsible for gastrostomy tube (G-tube) training for qualified staff. Provides information, support, and guidance to community members, other medical professionals, contractors, and staff regarding the specific needs of clients. Promptly notes and addresses gaps in skills, understanding, or compliance with staff and contractors. Communicates regularly with Program Coordinators, client's guardians, medical providers, and team members to ensure the customer's health and wellness needs are met. Complies with PPCH, State, Federal, and other applicable guidelines and regulations. Completes all required training, including annual refreshers, according to established timelines. Monitors, completes, and provides required documentation in a timely and accurate manner. Monitors QMAP staff activity to ensure protocols and regulations are followed. Promptly communicates and reports all behavioral incidents, medical emergencies, deficiencies in care provision, and/or significant incidents to appropriate agencies, teams, or individuals. Maintains knowledge and understanding of PPCH and other regulating agencies' policies, procedures, rules, regulations, and guidelines.
Other Duties:
Assists with internal or external audits as needed. Other duties as assigned.
Qualifications:
Education and Training:
Colorado LPN license in good standing is required. Developmental Disabilities Certification (DDC) preferred. Required training will be provided and must be successfully completed.
Experience:
Minimum of two years' experience working with people with I/DD required. Minimum of one year using Microsoft Office or other software required.
Knowledge, Skills, and Abilities:
Excellent written and verbal communication skills. Effective listening skills, even when non-traditional communication is used. Ability to direct others. Ability to respectfully address significant issues of health and safety while supporting an individual's choices. Knowledge of the Colorado Developmental Disabilities system, regulations, and rules preferred. Skill and ability to constructively receive and provide feedback. Ability and willingness to complete required trainings and learn PPCH operations and systems. Valid Colorado driver's license, and ability to meet PPCH's driving requirements.
Physical Requirements and Working Conditions:
Work will often be performed in a hybrid setting including private homes, a typical office setting, and occasionally in the community using a company provided laptop. This position involves occasionally physically assisting others, including pushing/pulling individuals using wheelchairs, transferring individuals with mobility needs, and assistance with daily living skills. Lifting, moving, and transferring up to 75 pounds. Regular lifting and carrying of approximately 15 lbs. May be required to bend, stoop, squat, and walk. Regular and frequent driving throughout the Denver Metro area required.
Pay Range:
