Remote Day Per Week Compensation:

$130,000 - $150,000 + RVU Bonus Opportunities Genesis Medical Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team providing primary care services to geriatric patients within a skilled nursing facility located in the Stamford area of New York's Catskills region. This role offers a hybrid structure that combines in-facility patient care with one remote workday per week dedicated to chart review, care coordination, telehealth follow-ups, and administrative responsibilities. The ideal candidate should be comfortable working independently and managing medically complex geriatric patients within the nursing home setting. Schedule 3 days per week working directly inside the nursing facility 1 remote workday per week Flexible scheduling available Providers must remain accessible by phone during working hours for patient care needs and coordination with facility staff Compensation Structure Base salary between $130,000 - $150,000 Additional RVU-based productivity bonuses Flexible compensation models available depending on experience and schedule Responsibilities Provide primary care services to geriatric patients within the skilled nursing facility Perform patient evaluations, follow-ups, admissions, and routine medical management Coordinate care with nursing staff, specialists, families, and facility administration Review labs, imaging, and clinical reports Complete documentation and EMR charting in a timely manner Utilize remote day for chart review, care coordination, telehealth follow-ups, and administrative responsibilities Requirements Active NP or PA license in New York Prior nursing home / SNF or geriatric experience preferred Strong clinical judgment and ability to work independently Comfortable managing medically complex patients EMR experience preferred Reliable transportation required This is an excellent opportunity for a provider seeking flexibility, strong earning potential, and long-term growth within a supportive and expanding medical group.
