This vital role involves delivering comfort, symptom management, and emotional support to patients facing life-limiting illnesses. As an On-Call Hospice Nurse, you will respond promptly to urgent needs, assess patient conditions, and coordinate care plans to ensure dignity and quality of life. This position offers flexibility and the opportunity to make a meaningful difference in patients' final stages of life while working within a supportive team environment. Responsibilities Respond to after-hours calls and urgent patient needs with professionalism, empathy, and promptness Conduct comprehensive patient assessments, including vital signs, symptom management, and physiological evaluations Collaborate with interdisciplinary teams to develop and modify individualized care plans that align with patient wishes and clinical best practices Provide medication administration, wound care, and symptom relief interventions in accordance with physician orders and hospice protocols Educate patients and families on disease progression, comfort measures, and available resources to support emotional well-being Document all patient interactions accurately in medical records while adhering to HIPAA regulations Coordinate discharge planning, post-acute care, and outpatient services as needed for seamless continuity of care Qualifications Valid nursing license with current registration in the state of practice Critical care experience preferred, including ICU or emergency room backgrounds Knowledge of infectious disease care, geriatrics, behavioral health, and post-acute care environments Strong understanding of medical terminology, physiology, vital signs assessment, and diagnostic evaluation techniques Proficiency in medical records documentation, discharge planning, utilization management, case management, and patient service delivery Ability to handle high-pressure situations with professionalism while demonstrating excellent patient assessment skills Experience with managed care settings or outpatient clinics is advantageous Join our team as an On-Call Hospice Nurse and bring your expertise to provide compassionate end-of-life care that truly makes a difference.
