About Us:

We are looking for a dedicated and skilled Registered Nurse (RN) to join our team in a per diem role. In this position, you will focus on supervising home care teams, conducting assessments, and evaluating patient care, ensuring the highest standards are met for all clients.
Job Overview:
As a Registered Nurse (RN) at Grays Home Care, your primary responsibility will be to oversee and evaluate patient care provided by home health aides and other support staff. You will conduct patient assessments, ensure care plans are appropriately followed, and provide guidance and support to our clinical teams. This is a supervisory and evaluative position, with no direct patient care involved. The flexibility of the per diem schedule allows you to balance work around your lifestyle.
Key Responsibilities:
Conduct Patient Assessments:
Perform initial and ongoing assessments of patients' health and care needs.
Supervise Home Care Teams:
Provide supervision, oversight, and mentorship to home health aides, ensuring care plans are followed effectively.
Develop and Evaluate Care Plans:
Collaborate with the healthcare team to develop individualized care plans. Monitor and evaluate the implementation of these plans to ensure they are achieving desired outcomes.
Quality Assurance:
Review and assess patient care to ensure compliance with industry standards and regulations.
Documentation and Reporting:
Maintain accurate and timely documentation of assessments, evaluations, and supervisory activities.
Training and Support:
Offer education and training to home care aides on patient care protocols and procedures.
Collaboration:
Work closely with physicians, family members, and other healthcare providers to address any changes in patients' conditions and adjust care plans as needed.
Qualifications:
Valid Registered Nurse (RN) license in North Carolina. Minimum of 2 years of nursing experience, preferably in home care or similar settings. Experience in supervisory roles or in leading a healthcare team is a plus. Strong assessment, critical thinking, and decision-making skills. Excellent communication and interpersonal skills. Ability to manage multiple priorities and work independently. Current CPR and BLS certification. Reliable transportation and valid driver's license. Why Join Grays Home Care?
