Job Description

a { text-decoration: none; color: #464feb; } tr th, tr td { border: 1px solid #e6e6e6; } tr th { background-color: #f5f5f5; } Join our team as a Transitional Care Management Quality Nurse and play a key role in advancing patient safety and quality outcomes across the continuum of care. In this role, you will provide timely telephonic outreach and clinical assessment within two business days of hospital discharge, ensuring safe care transitions and adherence to transitional care management requirements, while collaborating with providers, patients, and caregivers to reconcile medications, address post-discharge needs, and prevent readmissions through coordinated follow-up care. You'll leverage your clinical expertise to analyze data, support quality improvement initiatives, and identify trends that drive meaningful enhancements in care delivery. We invite compassionate, detail-oriented nurses who are passionate about patient-centered care, collaboration, and continuous improvement to apply. This full-time position will work Monday through Friday from 8:00am to 4:30pm. No holidays, evenings or weekends are required. This is a hybrid role so the selected candidate will have flexibility to work from home. Occasional travel to UPMC outpatient practices will be required, typically on a quarterly basis. Title and starting pay rate of this position may vary based on years of experience.

Reviews and extracts patient information from the medical record Participates in quality projects Aggregates/reports data based on project requirements Investigates and summarizes key issues based on quality case project Understands and maintains knowledge of regulatory and/or standards by actively reviewing and updating process to reflect the requirements. Promotes change and innovation that enhances the quality of patient care and the departmental functions and efficiencies. Demonstrates a passion for developing others, enthusiastically and expertly precepts students, graduate nurses and new hires. Analyzes clinical and behavioral situations, identifying learning opportunities to improve patient care practices. Regularly seeks feedback on performance and applies continuous professional improvement. Collects, prepares, and presents accurate, timely data to display results of process improvements that allow for trending and easy to understand. Reviews data and analyzes outcomes for patterns and trends in order to communicate findings related to standards, evidenced based practices, and policies for the improvement of processes. Demonstrates a passion for developing others, enthusiastically and expertly precepts students, graduate nurses and new hires. Analyzes clinical and behavioral situations, identifying learning opportunities to improve patient care practices. Regularly seeks feedback on performance and applies continuous professional improvement. Utilizes appropriate computer systems to obtain data, enter data and develop reporting tools.
