Erie, PA Job Details Full-time $33.80 - $56.74 an hour 1 day ago Benefits Work from home Qualifications RN License Employee relationship building Bachelor's degree Stakeholder relationship building Full Job Description Join our team as a Transitional Care Management Quality Nurse and play a key role in advancing patient safety and quality outcomes across the continuum of care. In this role, you will provide timely telephonic outreach and clinical assessment within two business days of hospital discharge, ensuring safe care transitions and adherence to transitional care management requirements, while collaborating with providers, patients, and caregivers to reconcile medications, address post-discharge needs, and prevent readmissions through coordinated follow-up care. You'll leverage your clinical expertise to analyze data, support quality improvement initiatives, and identify trends that drive meaningful enhancements in care delivery. We invite compassionate, detail-oriented nurses who are passionate about patient-centered care, collaboration, and continuous improvement to apply. This full-time position will work Monday through Friday from 8:00am to 4:30pm. No holidays, evenings or weekends are required. This is a hybrid role so the selected candidate will have flexibility to work from home. Occasional travel to UPMC outpatient practices will be required, typically on a quarterly basis. Title and starting pay rate of this position may vary based on years of experience.

Collects, prepares, and presents accurate, timely data to display results of process improvements that allow for trending and easy to understand. Reviews data and analyzes outcomes for patterns and trends in order to communicate findings related to standards, evidenced based practices, and policies for the improvement of processes. Bachelor's degree in Nursing or related field required, either at time of hire/transfer, or must be enrolled in a program within one year of hire/transfer date and completed within three years of enrollment. Minimum 2 years of nursing experience required. Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families. Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment. Exemplary critical thinking, analytical and problem solving abilities required as related to various aspects of patient care. Critical thinking skills necessary to exercise and to lead others. Mobility and visual manual dexterity. Physical stamina for frequent walking, standing, lifting.
Licensure, Certifications, and Clearances:
Registered Nurse (RN) Act 31 Child Abuse Reporting with renewal Act 33 with renewal Act 34 with renewal Act 73 FBI Clearance with renewal Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
