    Position Title:
    Adult Heart Post-Transplant Coordinator Accurate and efficient documentation of nursing care Knowledge and appropriate use of internal and external resources. This RN position is essential in managing patient care through the post-transplant stage and may also be involved in chronic disease management. Provide skilled observations, telephone triage, education, and health assessments of patients including history, appropriate interventions, and referrals. Foster continuity of care and facilitate transitions across outpatient and community providers on patient population as needed. Maintain knowledge of all applicable regulatory laws and requirements, including
    HIPAA, CMS
    Conditions of Participation, The Joint Commission, and UNOS to ensure regulatory compliance in nursing practice. Potential to support outreach efforts.
    Expectations:
    Critical thinking skills through the nursing process, excellent problem-solving skills, and demonstrated ability to set priorities. Customer service, interpersonal communication skills, and the ability to build respectful relationships within and between units and individuals. Adaptability to changing needs and priorities based on unit, department, and institutional goals/objectives. Knowledge and skills providing patient and family education and reinforcement related to transplant processes, including but not limited to telephone intervention and face-to-face meetings in both individual and group settings. Proficiency with typing and electronic health record documentation. Ability to manage complex clinical issues utilizing assessment skills, protocols, and evidence-based interventions. Required Qualifications
    • Current licensure to practice nursing in the State of Michigan.
    2 years of nursing experience as an RN within the last 5 years At least 2 years of experience in the past 5 years caring for patients in ambulatory care or home settings. At least 2 years of experience in the past 5 years caring for adult heart transplant recipients or candidates. At least 2 years experience within the last 5 years with education in lifestyle modification or health care regimen compliance. Transplant Certification (CCTC/CCTN) within three years of hire.

    • 2 of 5 years experience with ambulatory care telephone management of complex patients.
    2 years of experience in the last 5 years coordinating with multiple ambulatory care services for a single patient. 2 years of experience in the last 5 years working with chronic diseases, including renal failure, Diabetes Mellitus, transplant vasculopathy, or hepatitis.
