    Compensation:
    $57-$60 per hour (experience, education, location, and other factors.) πŸŽ“ Qualifications To be successful in this role, you must have:
    • Active RN License in NY State
    • Experience with UAS-NY or OASISmunity assessment tools
    • 2+ years of in-home care experience within the last 15 years
    • Experience with adult/geriatric populations; home care, disability services, ormunity-based care preferred
    • Strongputer skills andfort using new/emerging technology
    • Ability to conduct telehealth assessments
    • Excellent written and verbalmunication skills
    • Strong interpersonal andmunity engagement abilities
    • Knowledge of long-term care services and programs
    • Willingness to travel locally within assigned territory 🩺 Key Responsibilities In this meaningful clinical role, you will:
    • Reviewpleted NYS Uniform Assessment System (UAS)munity health assessments
    • Support physicians by ensuring accuratepletion of clinical orders
    • Conduct in-person follow-ups when needed
    • Review and make rmendations for long-term care services and supports
    • Use clinical judgment to validate assessment oues
    • Collaborate with interdisciplinary teams to close care gaps and improve quality
    • Provide clinical and educational guidance for program initiatives

    Pay Details:
