Location:
- Bronx, Brooklyn, Queens, & Manhattan, NY
Schedule:
- HYBRID (In-Home + Virtual)
Compensation:
- $57-$60 per hour (experience, education, location, and other factors.)
- Qualifications
- _To be successful in this role, you must have:_
- Active RN License in NY State
- Experience with UAS-NY or OASIS community assessment tools
- 2+ years of in-home care experience within the last 15 years
- Experience with adult/geriatric populations; home care, disability services, or community-based care preferred
- Strong computer skills and comfort using new/emerging technology
- Ability to conduct telehealth assessments
- Excellent written and verbal communication skills
- Strong interpersonal and community engagement abilities
- Knowledge of long-term care services and programs
- Willingness to travel locally within assigned territory
- Key Responsibilities
- _In this meaningful clinical role, you will:_
- Review completed NYS Uniform Assessment System (UAS) community health assessments
- Support physicians by ensuring accurate completion of clinical orders
- Conduct in-person follow-ups when needed
- Review and make recommendations for long-term care services and supports
- Use clinical judgment to validate assessment outcomes
- Collaborate with interdisciplinary teams to close care gaps and improve quality
- Provide clinical and educational guidance for program initiatives
- Demonstrate communication skills that support individuals across various age groups
