Job Description

PER DIEM REGISTERED NURSE
(RN) - HOME HEALTH Promise Home Health Care is seeking a compassionate and reliable Per Diem Registered Nurse (RN) to join our growing team.
Responsibilities:
Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments Develop and update patient care plans Coordinate care with physicians and interdisciplinary team members Complete accurate and timely documentation Provide patient and family education
Requirements:
Current California RN license Home health experience preferred Reliable transportation and valid driver's license Current CPR certification Knowledge of OASIS documentation preferred

Per Diem Pay:
$150.00 - $200.00 per hour
