Position Summary:
The Hospice Registered Nurse (RN) Case Manager is responsible for coordinating and managing the care of hospice patients and their families. This role focuses on providing compassionate, patient-centered care while ensuring quality, compliance, and interdisciplinary collaboration.
Key Responsibilities:
Conduct comprehensive patient assessments (physical, psychosocial, emotional, and spiritual) Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team (IDT) Coordinate care with physicians, social workers, chaplains, aides, and other team members Provide skilled nursing care, including pain and symptom management Educate patients and families about disease progression, medications, and end-of-life care Serve as the primary point of contact for patients and families Perform regular home visits and document care according to regulatory and agency standards Monitor patient condition and adjust care plans as needed Ensure compliance with Medicare, Medicaid, and hospice regulations Participate in IDT meetings and contribute to care planning discussions Provide emotional support and counseling to patients and families Coordinate discharge planning when appropriate
