Position Summary The Registered Nurse (RN) provides skilled nursing care to patients in their homes, focusing on promoting health, preventing complications, and supporting patients and families through chronic illness and end-of-life care. This role involves coordination with an interdisciplinary team to ensure compassionate, patient-centered care that aligns with individualized care plans. Responsibilities Conduct in-home patient assessments and develop individualized care plans. Administer medications, treatments, and wound care as prescribed. Monitor patient progress and communicate updates to physicians and care teams. Educate patients and families on health management and disease prevention. Maintain accurate documentation in compliance with regulatory standards. Collaborate with interdisciplinary team members to ensure continuity of care. Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations Develop, implement, and update individualized care plans in collaboration with physicians and the care team Provide skilled nursing care, including wound care, medication management, IV therapy, and symptom control Deliver hospice and palliative care focused on comfort, dignity, and quality of life Educate patients and families on disease processes, medications, treatments, and end-of-life care Monitor patient condition and communicate changes to physicians and appropriate team members Coordinate care with interdisciplinary team members (e.g., social workers, therapists, aides, chaplains) Maintain accurate and timely documentation in compliance with regulatory and agency standards Participate in on-call rotation as required Ensure compliance with infection control, safety, and quality assurance standards Qualifications Current RN license in TEXAS Graduate of an accredited nursing program Minimum of 1-2 years of clinical nursing experience (home health or hospice experience preferred) Valid driver's license and reliable transportation Current CPR certification (BLS)Apply Today Pay:
