About Us Carewell Hospice & Palliative Care is a CHAP-accredited, patient-centered hospice agency serving DuPage and surrounding counties. Our mission is to provide compassionate comfort care while supporting families during life's most challenging moments. As we continue to grow, we are seeking a dedicated Registered Nurse with hospice and marketing experience to join our team. Position Summary This hybrid role combines clinical nursing responsibilities with community outreach and referral development . The RN Clinical Liaison will conduct patient assessments and hospice admissions while also building relationships with hospitals, skilled nursing facilities, physicians, and case managers to educate them about hospice services and eligibility. Key ResponsibilitiesClinical Duties Conduct timely and compassionate hospice admissions and assessments Educate patients and families about hospice philosophy and care Collaborate with the interdisciplinary team (IDG) to ensure smooth onboarding Document all clinical encounters per regulatory guidelines Serve as a clinical resource for referral sources, families, and facility partners Marketing & Outreach Duties Build and maintain relationships with hospitals, SNFs, ALFs, memory care facilities, and physician offices Provide in-services to case managers, social workers, and nursing leadership Identify referral opportunities and support census growth Represent Carewell Hospice at community outreach events and facility visits Communicate our services, differentiators, and patient care approach with empathy Qualifications Active RN license in Illinois (Required) Hospice, home health, or palliative care experience (Preferred) Experience or comfort with healthcare marketing / clinical liaison work Excellent communication, presentation, and relationship-building skills Strong understanding of hospice eligibility (Medicare/Medicaid guidelines preferred) Valid driver's license and reliable transportation Benefits Competitive pay (salary or hourly + performance incentives) Flexible scheduling Supportive leadership team Mileage reimbursement Paid training and continuing education opportunities Growth opportunities as the agency expands
