Overview Join our dynamic healthcare team as a Registered Nurse providing PRN (as-needed) home health visits! In this vital role, you will deliver personalized, high-quality nursing care directly to patients in their homes, promoting health, safety, and independence. You will be an essential part of a compassionate team dedicated to delivering exceptional patient-centered care across diverse communities. Responsibilities Conduct comprehensive patient assessments including vital signs, physical examinations, and health histories to determine individual care needs. Administer medications, injections, IV infusions, and perform wound care, ostomy management following aseptic techniques. Provide specialized care for patients with complex conditions including dementia, Alzheimer's disease, developmental disabilities, and memory care needs. Educate patients and their families on disease management, medication administration, nutrition, and safety protocols to promote optimal health outcomes. Coordinate with interdisciplinary teams to develop and implement personalized care plans aligned with patient goals and medical directives. Document all patient interactions accurately using electronic health record (EHR) systems such as Wellsky and Kinnser while ensuring compliance with HIPAA regulations. Manage urgent situations through triage skills, including acute pain management, airway management, and responding effectively to emergencies during home visits. Requirements Valid RN license in the state of practice with current CPR/BLS certification. Proven experience in diverse settings such as hospital medicine, critical care (ICU or trauma centers), medical-surgical units, or home health environments. Familiarity with EMR/EHR systems like Wellsky Kinnser for efficient documentation and communication. Strong clinical skills including IV insertion, phlebotomy, medication administration, catheter care, and laboratory specimen collection. Knowledge of infectious disease control practices and sterile processing procedures. Experience working with individuals with disabilities or in geriatrics, memory care, hospice & palliative medicine preferred. Ability to travel within assigned service areas; valid driver's license required. Excellent communication skills combined with compassionate patient care approach; ability to work independently while collaborating effectively with healthcare teams.
