CPR Full Job Description Job Description:
We are seeking a compassionate and experienced Registered Nurse (RN) to provide nursing oversight and support for individuals receiving services through the Home and Community-based Services (HCS) Program in Host Home/Companion Care settings. The RN will ensure compliance with HCS regulations, coordinate healthcare services, monitor health outcomes, and support individuals in achieving the highest level of health and independence.
Essential Responsibilities:
Conduct nursing assessments and health screenings for individuals enrolled in the HCS program. Develop, review, and update nursing service plans and health-related documentation. Monitor medication administration and provide medication training as required. Coordinate medical appointments and communicate with physicians, specialists, pharmacies, and other healthcare providers. Provide health education and training to Host Home providers, direct support professionals, and team members. Assess and respond to health concerns, changes in condition, and medical emergencies. Participate in interdisciplinary team meetings and person-directed planning processes. Ensure compliance with HCS program standards, state regulations, and agency policies. Maintain accurate, timely, and confidential documentation.
Qualifications:
Current, unrestricted Registered Nurse (RN) license in the State of Texas. Minimum of one (1) year of nursing experience; experience with intellectual and developmental disabilities preferred. Knowledge of Home and Community-based Services (HCS) regulations preferred. Strong assessment, communication, and documentation skills. Ability to work independently and manage a flexible caseload. Valid driver's license, reliable transportation, and current auto insurance. Current CPR certification.
Preferred Qualifications:
Experience working with individuals with intellectual and developmental disabilities. Familiarity with person-centered planning and community-based service delivery. Experience in case management, community health, or long-term support services.
