Monday-Friday:

8:00 AM - 8:00 PM (On-Call) Position Overview We are seeking a compassionate and dependable Visiting After Hours Registered Nurse (RN) to join an after-hours and comfort care team serving the Loveland, CO area. This role combines work-from-home triage responsibilities with in-home patient visits during evenings and weekends as clinically necessary. Scheduled home visits will be required during daytime hours on Saturdays and Sundays, with additional evening home visits Monday through Friday. Outside of scheduled visits, the RN will provide after-hours triage/on-call support and conduct in-person visits when deemed medically necessary. In this role, you will respond to after-hours calls from participants, families, caregivers, and staff, utilizing strong clinical judgment to assess needs and determine appropriate interventions. This may include telephonic support, care coordination, escalation to providers, emergency response, or in-person nursing visits. This position supports participants enrolled in a PACE program and collaborates closely with interdisciplinary teams and primary care providers to ensure high-quality, participant-centered care. Key Responsibilities Provide after-hours phone triage and clinical assessments Conduct scheduled and urgent home visits as medically necessary Perform daytime home visits on Saturdays and Sundays Complete evening home visits Monday through Friday as assigned Assess symptoms telephonically and in person to determine appropriate level of care Coordinate care with physicians, interdisciplinary teams, and community resources Educate and support participants and families, including end-of-life care discussions Initiate emergency response when appropriate Perform comprehensive nursing assessments and follow established care plans Delegate and prioritize tasks appropriately during on-call shifts Support care transitions including respite, discharge, and facility transfers Complete timely and accurate EMR documentation Participate in team huddles and occasional in-person meetings as requested Required Qualifications Active Colorado RN License Current BLS & First Aid certification (American Heart Association or American Red Cross) Minimum 2+ years of healthcare experience with a focus in geriatrics At least 1 year of home health experience Reliable transportation, valid driver's license, clean driving record, and auto insurance If you are an experienced RN who thrives in independent, critical-thinking roles and values providing compassionate after-hours care, we encourage you to apply today.
