Pinnacle Home Care, Florida's largest independent Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we're looking for Registered Nurses to join our award-winning team. Key Responsibilities Design and implement patient-specific care plans focused on achieving optimal recovery and enhancing overall health. Provide high-quality nursing care in alignment with the patient's care plan, including comprehensive health and psychosocial assessments to support recovery. Monitor patients' conditions and coordinate health promotion, prevention, and direct care. Maintain detailed and accurate clinical records, including all required OASIS documentation. Communicate effectively with clinical managers and colleagues to ensure high-quality care. Provide training and support to patients and their families/caregivers. Function as a key point of contact between patients and community health resources. Work alongside physicians and discharge planners to provide health education to patients and caregivers. Confirm daily visit schedules to ensure seamless coordination with other staff. Demonstrate awareness of the agency's Quality Assurance and Performance Improvement (QAPI) program and participate as required. Understand and adhere to the agency's process for managing and addressing customer complaints. Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency's service agreements and admission policies. Understand the agency's policies regarding appropriate patient discharge and reasons for discharge.
