Part-Time 2nd Family is hiring a Part-Time Registered Nurse (RN) Care Coordinator to support senior clients and the caregivers who serve them. This role is ideal for an RN who values flexibility, autonomy, and meaningful relationship-based care. The RN Care Coordinator plays a central role in ensuring clients receive thoughtful, high-quality non-medical home care. You will assess client needs, develop care plans, supervise caregivers, and serve as a trusted nursing point of contact for families. Most work can be done remotely from home, with limited in-person responsibilities for client assessments, caregiver orientations, and occasional local networking events. What You'll Do As the RN Care Coordinator, you will: Conduct client assessments and care planning by completing initial in-home consultations, 90-day reassessments, nursing assessments, and individualized care plans. Manage ongoing client care by monitoring progress, maintaining clinical documentation, addressing family concerns, and helping ensure our Grandma Guarantee standard is consistently met. Supervise and support caregivers by providing direction, instruction, mentorship, and client-specific guidance to CNA caregivers delivering non-medical care. Serve as the primary nursing contact for clients and families , answering questions, documenting communications, and providing education related to medications, conditions, plan of care, and individualized support needs. Maintain on-duty availability from 7:00 AM to 7:00 PM , responding to client calls and emails within 30 minutes or less during duty hours. After-hours communication is limited to emergencies. Work with scheduling flexibility to complete initial assessments , typically within 24 to 48 hours of request, and collaborate professionally with fellow RN Care Coordinators as needed. Support team operations by attending monthly company meetings , conducting caregiver orientations when needed, and assisting with reasonable care-related or regulatory requests within the role's scope. In-Person Expectations Although this is primarily a remote position, some in-person work is required, including: Scheduled in-home client assessments Quarterly reassessments (minimum of 4 in-person assessments per client each year) Weekly caregiver orientations Occasional attendance at local community and professional networking events Qualifications To be considered, candidates must have: Current RN license in the State of Texas At least 2 years of experience as a Registered Nurse Strong ability to build positive, professional relationships with clients, families, and staff Experience or skill in caring for the elderly population Excellent oral, written, and documentation skills Strong organization, leadership, and problem-solving abilities Professional demeanor and appearance Preferred Qualifications Preferred candidates may also have: Experience in hospital, rehabilitation, skilled nursing, hospice, or assisted living settings Established professional referral relationships and community connections Interest in helping grow our client base through professional networking Spanish-speaking ability, though not required Compensation Compensation is paid bi-weekly and includes: Case management: $100 per case Initial in-person assessment: $50 per client 90-day reassessment: $50 per client Caregiver orientation: $50 per session Why Join 2nd Family At 2nd Family, we believe home care should feel personal, compassionate, and deeply trustworthy.
