With a broad network of sites and services, we focus on mental health, addiction recovery, and community outreach, including specialized older adult support programs. Overview We are seeking a passionate and energetic Registered Nurse specializing in Senior Services Outreach to join our Senior Assistance for Independent Living (SAIL) Case Management Program. In this vital role, you will serve as a key advocate for elderly clients, providing medical education, health assessments, and care coordination to older adults living independently in the community. Your expertise will help bridge healthcare gaps and foster meaningful connections with seniors and their families. Duties Respond to referrals and inquiries from the Morris County Office on Aging SAIL Case Managers for in-home health and wellness evaluations. Conduct Comprehensive In-Home Physical and Mental Health Assessments to evaluate physical health and functional status, mental health and cognitive functioning, environmental safety, psychological, social, and emotional well-being, and other conditions that may jeopardize an individual's ability to remain living independently. Provide health education, nursing support, and preventive guidance tailored to older adults and their caregivers. Collaborate as an active member of an interdisciplinary treatment team , contributing clinical insight to care planning and service coordination. Maintain timely, accurate, and compliant documentation. Work Environment & Schedule 35 weekly hours Majority of work is conducted in the field with community-based home visits Flexible work hours with supervisor approval. Service documentation may be completed in the field or from home using an agency-issued device. Travel throughout Morris County required; mileage reimbursement provided for use of personal vehicle Requirements Active Registered Nurse (RN) license in the State of New Jersey Valid driver's license and reliable personal vehicle Experience working with older adults in environments like assisted living, nursing homes, or memory care facilities. Ability to work independently while collaborating effectively with a multidisciplinary team Comfort working with older adults in community and home-based settings Preferred Experience in community health, geriatrics, case management, home health, or social services Knowledge of aging services, community resources, and senior provider networks Join us in making a difference through compassionate nursing care!
