Job Description

About Us:
We are seeking a compassionate and self-motivated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Home Health team. In this role, you will provide nursing care to clients in their homes, focusing on individualized treatment plans, patient education, and promoting independence. The ideal candidate has strong clinical judgment, excellent time management, and the ability to work independently in the field.

Accountability Key Responsibilities:
Perform in-home assessments and develop individualized care plans in collaboration with physicians and other healthcare providers. Proper administration of prescribed treatments, medications, and therapies. Calling in refills for medications and sometimes picking them up from the pharmacy. Monitor patient condition and report changes in health status to the physician and clinical team. Educate clients and caregivers on disease management, medication adherence, wound care, and safety in the home. Maintain accurate and timely documentation and charting. Coordinate care with interdisciplinary team members, including office staff, case workers, and home health aides. Ensure compliance with federal, state, and agency regulations and standards. Consistently demonstrate competency with technical nursing skills according to legal scope of practice.
Qualifications:
Education:
Graduation from an accredited school of nursing and one-year clinical experience within the past five years or completion of an accredited refresher course within the past two years.
Licensure:
Current State RN license without encumbrances and in good standing.
Certifications:
Current CPR/BLS certification. Reliable transportation, valid driver's license, and auto insurance Knowledge and ability to provide skilled nursing care as defined in the State Nurse Practice Act. Available for work on weekends and holidays as required. Read, write, speak and understand English as needed for the job. Meets current health requirements to provide patient care. Participate in in-service education programs totaling two hours per year, or as State law mandates, whichever is greater. Assumes responsibility for professional development and contributes to the professional growth of others.
Skills:
Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Proficient in using home health documentation systems (e.g., Generations, Google Drive) Ability to work independently and manage a flexible, mobile schedule
Physical Requirements:
Must be able to lift up to 50 pounds and assist with patient mobility Requires walking, standing, driving, and entering/exiting patient homes regularly
