Job Description
Position Summary We are seeking a dedicated and collaborative Care Management Nurse to join our growing care management team. This role is ideal for a nurse who values teamwork, open communication, and active engagement with colleagues to deliver high-quality, patient-centered care. Key Responsibilities Collaborate closely with the interdisciplinary care management team to coordinate and optimize patient care. Actively participate in team meetings, case conferences, and care planning discussions. Provide comprehensive care management services, including assessments, care coordination, patient education, and follow-up. Communicate effectively with patients, families, providers, and internal team members. Document care plans and patient interactions accurately and timely. Support team goals by sharing insights, best practices, and contributing to continuous improvement initiatives. Maintain compliance with regulatory, organizational, and professional standards. Team Collaboration Expectations Demonstrated ability and willingness to work as a team player within a collaborative care management environment. Openness to feedback, shared decision-making, and peer support. Commitment to engaging with the broader care management team to achieve patient and organizational outcomes. Work Schedule & Training Hybrid position : combination of remote work and on-site presence. All onboarding and training will be conducted in person to ensure a strong foundation, effective collaboration, and connection with the care management team. Ongoing in-person attendance may be required for meetings, training updates, or team activities. Qualifications Active RN license in good standing. Previous experience in care management, case management, or a related clinical role preferred. Strong communication, organizational, and interpersonal skills.
