Nurse Case Manager II (RN) Compensation Competitive pay rate: $44.14 per hour Nurse Case Manager II (RN) Highlights Monday-Friday, 8:00 AM - 5:00 PM EST No nights, weekends, holidays, or on-call requirements Field-based and telephonic case management role Fully remote when not conducting field visits Contract opportunity with potential for long-term career growth Individual start opportunity Nurse Case Manager II (RN) Responsibilities Conduct comprehensive assessments of members' health needs and eligibility Develop, implement, and monitor individualized care plans Coordinate care and advocate for appropriate healthcare services Support members with chronic conditions such as diabetes, hypertension, and asthma Perform crisis intervention and follow-up for members experiencing medical or behavioral health crises Collaborate with healthcare providers and multidisciplinary teams to improve outcomes Utilize motivational interviewing and coaching techniques to encourage healthy lifestyle changes Analyze clinical, utilization, and self-reported data to identify member needs Maintain accurate documentation within case management systems Complete telephonic outreach and field visits as needed Nurse Case Manager II (RN) Requirements Active, unrestricted RN license in Virginia or Compact State License required High School Diploma or GED required Minimum 3 years of clinical nursing experience required Healthcare and/or managed care industry experience required Experience in hospital, home health, ambulatory care, or similar clinical settings required Strong computer skills and ability to navigate multiple systems Excellent verbal and written communication skills Ability to work independently and manage multiple priorities Must reside in Virginia Dedicated, distraction-free home office required Preferred Qualifications Case Management experience CCM Certification Experience managing chronic conditions Previous experience working with health plan members and care coordination programs If you think this Nurse Case Manager II (RN) position is a good fit for you, please reach out!
