Job Description

RN Case Manager, Full-time, 7:30am - 4:00pm Western Reserve Hospital - 3.5 Cuyahoga Falls, OH Job Details Full-time 3 hours ago Qualifications CPR Certification RN License BLS Certification Medical treatment knowledge Physical assessments Full Job Description Facilitate the patient encounter with the hospital, specifically in reference to the insurance process, discharge planning, and appropriate utilization of resources. May service as palliative care services advocate for patients as appropriate. Provides education for healthcare professionals, patients and their families. Participates as a member of the case management team.
Experience:
Three (3) years patient care experience encompassing patient assessment, treatment, lab values, etc with current clinical knowledge. Palliative care experience may be preferred.
Schedule:
Monday - Friday 7:30AM - 4:00PM, with rotating weekend/evening call (via phone only). Could potentially become hybrid remote position after orientation is completed.
