The Home Care Case Manager RN is responsible for independently coordinating and managing patient care in the home setting, ensuring each client receives high-quality, individualized services tailored to their needs, goals, and environment. This role blends clinical expertise, critical thinking, and direct patient engagement in the field. You will be the driving force behind consistent, patient-centered care assessing needs, implementing care plans, and making real-time clinical decisions that directly impact outcomes and quality of life. As a key part of our clinical team, you ensure continuity of care, maintain accurate and timely documentation, and uphold all regulatory and quality standards while supervising and delivering compassionate care in the community. Responsibilities Manage a designated caseload across Kane County while maintaining strong clinical oversight and engagement with each client Perform comprehensive in-home assessments, including physical evaluations and medical history reviews, to determine care needs and develop individualized, goal-oriented care plans Deliver skilled nursing care, including wound care, IV therapy, home infusions, injections, and medication administration as prescribed Educate, counsel, and support clients, families and field employees to promote understanding, safety, and improved health outcomes Serve as the primary clinical point of contact for clients, families, field employees, and other involved parties Provide ongoing health instruction and guidance tailored to patient and family needs Care Coordination & Clinical Oversight Instruct, supervise, and evaluate care provided by license and non-licensed staff through in-home supervisory visits and reassessments every 60 days Ensure continuity of care through ongoing monitoring, communication, and clinical evaluation Maintain accurate, timely, and compliant documentation in an electronic medical record (EMR) system Ensure all clinical records meet local, state, and federal regulatory requirements, including HIPAA compliance Quality & Professional Practice Participate in agency performance improvement and quality initiatives Stay current with best practices, clinical standards, and updates in home health nursing Contribute to a culture of safe, compassionate, and evidence-based care delivery Benefits Weekly pay with direct deposit Competitive starting salary of $82,000 annually, plus incentive bonus opportunities Potential for advancement into an Assistant Director of Nursing position 401(k) retirement savings plan Health insurance: medical, dental and vision Gas and cell phone allowance Electronic device with internet access for documentation on BrightStar Care's proprietary software system and air card for internet access Free continuing education credentialing Security of working for a Joint Commission accredited agency Requirements Current unrestricted RN license in Illinois BSN Preferred Minimum of one (1) year experience in home health and/or or case management experience.
